The following items are required to be submitted for each promotion candidate.

1. Check List
2.  Valid GUP/DAN card (please make sure DAN membership has not expired)
3.  4 Passport Pictures
4.  2 copies of Essay

     Cho Dan: 1,000 words
     E Dan: 2,000 words
     Sam Dan: 3,000 words

5.  Completed WTSDA Building Fund Donation Form
6.  WTSDA DAN Test Promotion Form
7.  WTSDA DAN Membership Form (if Cho Dan Bo)
8.  Fees – PLEASE WRITE POST-DATED CHECKS USING THE TEST DATE 

Please write two checks; one check payable to “WTSDA” for the Test Fee and Dan Membership Fee combined.  The second check payable to “WTSDA Foundation” for the brick donation.

Please note we will not accept any scanned/email copies of the above items.  All items need to be mailed to Master Dave Mukherjee at the following address.

There will be a $10 per student late fee (payable to WTSDA Region 4 Membership Fund) if not mailed by the requested date which is communicated to the studio owners per test cycle via email. Paperwork will not be accepted on the day of the testing.  In addition, the Region will supply the breaking materials.
